(00:00) 너무 막 집착하고 이러면은 그냥 탄력성이 떨어져 가지고 흔들흔들 해지죠 우리가 그 힘을 좀 키우고 그걸 개교해서 다음번에 올 수 있는 불행을 잘 극복할 수 있도록 긍정적인 생각을 좀 가지시면 훨씬 낫지 않을까 이런 생각이 듭니다 교수님이 조염병 강박증 분야 군내 최고 권위자의 세계적인 내 영상화 전문가 분이셔서 요즘에 또 마음이 아프신 분들 또 그 마음이 아파서 일어나는 여러 가지 사건 사고들이 많 다 보니까 또 그리고 저도 이제 사실 우울증과 조현병에 관해서 관심이 워낙 많기도 하고 가족으로 이제 그런 일을 겪은 것들을 이제 저희 구독자분들 아셔서 뭐 관련해서 좀 여쭤보고 또 교수님의 의견을 들어서 우리가 좀 나을 수 있는 방법 여쭤보려고 하는데요 증부의 왜 걸리고 몸이 보내는 우울증의 그 증조 할까요 어떤 신호들이 좀 있을까요 그 우울증은 그냥 한마디로 말하면은 뇌 기능이 떨어져 있는 겁니다 뇌기능이 우울한 거죠 기분 우울하고 뭐 이러면 모든게 다 활력도 떨어지고 기운도 없고
(01:04) 그러잖아요 우리가 우울하다 그러면 그 뇌가 우울한 겁니다 우리 생 디는 마음이 우울하다 얘기하는데 그 마음은 정확하게 뇌인 건가요 그렇죠 마음이 뇌에서 나오기 때문에 과거에는 이제 마음이 심장에 있다 뭐 마음심 그러니까 심장인데 실제로는 뇌에서 나옵니다 모든 인간의 정신 현상 심리적인 거 행동 뭐 이런게 다 뇌가 주관을 하기 때문에 우울 정도 뇌가 우울한 거죠 실제로이 뇌 영상을 찍어 보면 그 뇌가 일반인보다 좀 이렇게 활력이 떨어지고 달라요 다르죠 우울하죠 우리가 마음이 우울하다 실제로 뇌가 활력이 떨어지고 활성도가 떨어지는 그게 동전의 앞뒷 면입니다 같은 현상을 이쪽에서 보면은 마음이 우울하다 이렇게 보이는 거고 이쪽에서 보면 그 마음이 우울한 것과 똑같이 변화가 되는 뇌 뇌가 우울해져 있는 거죠 네 그래서 우울한 여러가지 이제 그런 원인이 있지 않겠습니까 그게 이제 우울의 원인이 되는 거죠
(02:07) 그러니까 우울증은 굉장히 많습니다 우리가 뭐 잠을 오래 잔다 한 15시간 16시간 계속 잔다 그러면은 잠을 자면은 이게 활성도가 떨어지죠 그렇게 하면 좀 의욕이 없어지죠 그 똑같습니다 우리가 막 움직이고 활동하고 운동하면 그게 뇌를 작아 가니까 우울 증상이 좀 나아지고 결국은 뇌의 현상이 이런 걸 반영을 한다 그렇게 생각을 하시면 되죠 그럼 내 기능이 우울하고 내 활력이 떨어진다 해주셨는데 요거는 왜 우울하고 왜 떨어지는 건가요 그냥 뭐 우리가 우울증이 하면 감기처럼 앉는 우울증 그런 거는 이제 우리가 생각하 의학적으로 약을 먹어야 정도까지 아닌 경우가 많잖아요 약을 먹을 정도까지 우울증이 심하신 분들은 왜요 그냥 뭐 잠 좀 많이 잤다고 그냥 안 움직였다고 병원가 정도의 우울증이 그러진 않잖아요 일단 그 우울 증상은 넓게 뭐 여러 가지 원인에 의해서 올 수가 있고요 이제 병적인 우울증 주요 우울증 이렇게 이야기를 합니다 주요 우울증 영어로는 mdd 그 병적인
(03:12) 우울증은 이제 기준이 있습니다 그데 약간 우울하다고 뭐 병적인 우울증이다 이렇게 하기는 어렵고요 그 우울증의 기준이 있는데 그게 이제 그냥 일반적으로는 우울증이 한 2주 이상 계속이 되면이 우리가 그냥 우울한 원인이 있어서 우울한 경우는 그 우울한 본인이 금방 없어지면 되게 이제 좋아지죠 예를 들면 학생인 경우에 뭐 오늘 시험을 쳤는데 시험을 잘 못 쳤다 그럼 우울하죠 근데 그거는 이제 뭐 하루 이틀 지나면 금방 좋아지거든요 그데 그런 거는 이제 반응성 우울증 외부에 원인이 있기 때문에 그거에 대한 반응을 우울하다 그런데 이제 병적인 우울증은 그런 것과 관계없이 약간 이렇게 우울증이 걸릴 수 있는 체질이라는 스트레스에 약하다 외부에 자극이 심하게 온다든지 또는 자극이 없더라도 어떤 계기에 의해서 우울해지는 거죠 그래서 우울 증상이 오래가고 또 거기에 따라서 여러 가지 다른 증상들이 나타납니다 주요 우울증이라는 우울한 기분만 있는게 아니고 따라서 신체적인 증상이 이렇게 나타나죠 잠을 많이 잔다든지 또는 어떤 경우는 잠을
(04:15) 거의 못 잔다든지음 맞아 맞아 많이 먹는다든지 어 여성분들이 좀 우울하면 많이 먹잖아요 그럼 기분이 좋아져요 그 많이 먹는다든지 또는 어떤 분들은 거의 먹질 않해요 우울하기 때문에 그리고 이제 의욕이 없어지고 죄책감도 들고 별로 잘못한 것이 없는데 본인이 우울한 우울하고 자기가 잘못한 것처럼 느끼는 거예요 죄책감이 들고 그러다가 이제 심해지고 이러면 아 이거 살아서 뭘 하나 의미가 없다 어 그냥 뭐 죽는게 낫겠다 이런 이제 자살 사고도 있고 그런 여러 가지가 이제 동반되어 오는 거죠 근데 지금 방금전 체질 얘기해 주셔서 그런 거에 좀 권리 확률이 높은 체질인 사람의 어떤 특징 같은 건 뭐가 있나요 어 한마디로 말씀드리기는 좀 사실 어렵습니다 그런데 되게 우울 정상을 좀 아주 느끼고 이런 분들은 성격적으로 좀 내성적인 경우가 많고 내성적 친구 관계가 그렇게 많지 않고 또는 혼자 지내기 좋아하고 이런 경우가 약간 우울증에 걸리기가 좀 쉬운 체질이고 근데 뭐 꼭 그렇다고 해서 모든 사람이 우울증이 걸린다 그런 건
(05:19) 아니죠 가장 중요한게 대인 관계를 가지는 것 또는 성격이 좀 외향적인 것 이런 두 가지가 굉장히 중요하다고 돼 있습니다 모든 정신질환 또는 뭐 정신 질환뿐만 아니고 일반인들이 살아가는데 있어서 자기가 어떤 뭐 좀 긍정적인 생각을 자꾸 가지게 되고 또 행복감을 느끼게 되고 이런 거를 관련하는 데는 그 두 가지가 그 대인 관계가 좀 많고 친구 관계가 좀 좋은 사람 그다음에 성격이 외향적인 사람 어이 두 가지는 어떤 영향을 미치길래요 쉽게 이제 회복될 수가 있죠 예를 들면 우울한 일이 있더라도 좀 우울해지다 그래도 그 내성적인 사람은 본인 혼자 그냥 그걸 극복을 하려고 짧고 그러잖아요 그렇지만 약간 친구 관계가 좀 좋고 많고 이런 사람들은 친구들이 또 용기를 복돋아 주기도 하고 관계에서 그래서 그런게 굉장히 중요합니다 그리고 이제 병적인 우울증까지 안 가지만 우울한 경우 그런 경우에 내가 기능이 떨어져 있기 때문에 그거 이제 기능을 좀 올리는 활동을 하면 또 좋아지죠 좀 전에 말씀드린 친구가 좀 용기를 북두사
(06:22) 주는 말을 했다 또는 책을 읽었다음 그래도 약간 뭐 우울한 거는 좋아질 수 있죠 그리고 이제 뭐 운동을 한다 운동을 하면 뇌가 뇌신경 세포가 활성화 되니까 우울 정상은 좋아지죠음 그 이게 결국은 우울한게 이제 뇌의 기능인데 약간 약한 정도는 일반적인 우리 여러 가지 방법을 통해서 좀 좋아질 수가 있고 병적인 우울증은 좀 치료가 필요한 상황이 되겠죠 그냥 한 며칠 정도 우울한 이런 관계 같은 우울증에서 그 모든 사람들이 병적인 우울증으로 가는 건 아니잖아요 저도 나중에 한번 겪은 적이 있으니까 깨달은 거는 와 이거 초기에 빨리 잡고 안 잡고가 계속 평생을 안고 사느냐 안 사느냐 결정할 수 있다는 생각이 들더라고요 어떤 사람이 좀 빨리 그래도 이런 사람은 병원을가 보는게 좋다 있을까요 어 일단은 우울증이 정상이 좀 지속이 된다면 병원에 가서 도움을 받는게 낫겠죠 그러니까 일반적으로 우울한 원인이 외부에 있어서 그냥 약간 약한 정도의 우울증이 짧은 기간 동안 그런 경우는 혼자 극복할 수 있지만 그게 어느 정도 지속이 되면 더 지속이 되다가
(07:22) 예를 들면 뭐 일주일 이상 지속이 되고 2주일 가고 이러면 병적인 우울증일 가능성이 많겠죠 아무래도 또는 그 그 우울 증상의 깊이가 깊다는 몇가지 가서 물건 좀 사고 뭐 이렇 하다 보면 기분이 좋아지는 뭐 그런 거는 뭐 일방 일상적으로 그게 아니고 좀 깊은 경우 그냥 움직이기도 싫고 그냥 계속 누워 있고 식욕도 없고 그게 또 지속이 되면서 아 이거 뭐 내가 이렇게 살았어 뭐 의미가 있나 죽고 싶은 생각이 막 들고 이러면은 빨리 좀 도움을 받아야죠이 우울증은 어떻게 보면 긍정적인 측면이 많습니다 인간이 어떻게 우울해 보지 않고 인생을 알겠습니까 당연히 우울한 여러 환경이 있을 거고 살다 보면은 뭐 행복한 경우만 없고 우울한 경우가 더 많을 수가 있죠 불행도 다 그렇게 그걸 안고 살아가는게 우리 인생이지 않습니까 그런데 그런 걸 통해서 우리가 조금 더 성숙해지고 좀 더 자기를 되돌아볼 수 있고 그런 기회가 되죠 그데 상당히 긍정적인 측면이 있는데 그게 이제 너무 떠나 가지고 더 깊이 이제 막 우울해져서 좀 안
(08:25) 좋은 일이 생긴다 이런 것은 조금 이제 빨리 도움을 받는게 나을 고 또 뭐 약한 정도의 우울증상은 뭐 혼자 극복할 수가 있겠죠네 자 그럼 우울증 극복하는 방법을 아까전 대인 관계도 중요하고 외향적인 거 그다음에 운동도 중요하다 말씀해 주셨는데 병원에 찾아갔을 때 의학적인 도움을 받았을 때는 어떤 것들을 우리가 좀 극복할 수 있는 거죠 일단 이제 우울 정상이 심하면 약을 일단 사용을 하는게 좋습니다 우울을 한 그 기간을 잘 넘겨 넘겨야 되기 때문에 우리 뇌가 우울하다고 제가 말씀드렸는데 실제로 그걸 조금 전문적으로 말씀드리면 우리 뇌는 신경 세포로 되어 있습니다 그래서 신경 세포의 연결이 우리 뇌라고 볼 수 있는데 그 신경 세포와 신경세포의 연결이 그 사이에는 갭이 있습니다 바로 이렇게 붙어 있는게 아니고 갭이 있는데 갭이 있기 때문에 한 그전에 신경세포에서 정보가 와 가지고 그다음 신경세포에 정보가 전달이 되려면 간격이 있기 때문에 정보가 전달이 안 돼요 그래서이 정보가 전달이 되려면 정보를 전달하는 물질이 필요한 거예요 그걸 이제
(09:29) 신경전달 물질이라고 우리 뇌에는 신경 전달 물질이 한 200가지 이상 됩니다 그중에 중요한게 한 열가지 정도 되는데 뭐 여러분들이 아마 많이 들어보셨을 건데 도파민 도파민 세로토닌 예 노아드레날린 뭐 이런 이야기를 들어보셨죠 그중에 이제 우울증과 관련이 있는 거는 세로토닌과 노 아드레날린이 좀 주로 관련된다 어 그 신경 전달 물질이 좀 부족하다 쉽게 말씀드린 뇌에서 좀 부족하다 이게 우울증입니다 그래서 약을 사용을 하면 그 약이 그 전 신경세포에서 물질이 나와서 그 신경세포의 수용체 달라붙습니다 그 도파민 신경 전달 물질이 도파민 수용체에 달라 붙겠죠 그렇게 딱 되어 있는데 세로토닌은 세로토닌 신경세포 수용치 달라붙어서 그다음 정보를 전달합니다네 그런데 약을 사용을 하면이 신경 전달 물질이 그다음 신경세포에 수용체에 붙어 있다가 떨어져서 다시 원래 자리로 로 재흡수가 됩니다 한번 사용하고
(10:32) 없어지는게 아니고 재흡수가 돼서 재사용을 합니다 그게 효율적이겠죠 재흡수를 하는고 길을 차단해 버립니다 약물이 그러면 어떤 일이 벌어지면 세로토닌이 제업 away 돼서 빨리 그 신경 세포에 세로 사용이 돼야 되는데 그게 안 되고 그 간극 우리가 시냅스 하는데 시냅스에 오랫동안 세로토닌이 머물러 있게 됩니다네 그래서 세로토닌 신경계가 활성화 같어 되는 거죠 그래서 우울증이 좋아진다음 이렇게 말씀드릴 수가 있는데 그렇다면 우울증은 세로토닌 신경세포의 기능이 떨어져 있는 거지 않겠는가 그렇게 우리가 유치해 볼 수가 있는 거죠음 그런 약물 치료를 하게 되면 대부분 많이 났나요 어 지금 통계적으로 보면 병원에 와서 울증을 치료받는 사람이 약물 치료를 하면 한 2분 정도가 좋아진다고 되 있습니다 그리고 약물이 아닌 가짜 약물 가짜 약을 사용하죠 네네네 그걸 주면 1 정도가 좋아진다고 합니다 어 그 무슨 효과가 있잖아요 용어가 플러스 보 예 플라보
(11:36) 플러스 효과죠 일종에 밀가루 밀가루전 밀가루 그걸 주면서 이거 약이야 이렇게 주면 3분 좋아지는 거예요 그러니까 결국은 우울증 약과 가짜약 플라세보 차이는 1이라고 할 수가 있죠 병원에 온 우울증 환자가 전체 100% 1분 정도는 항우울제로 좋아지고 1분은 항울제를 안 쓰고 그냥 가짜약을 써도 좋아지고 네은 약을도 좋아지지 않는다 좀 시간이 걸리고 여가지 어떻게 보면은 1의 효과가 약물 효과라고 볼 수가 있고 그런 제 일종에 메타 연구가 나와서 상당히 좀 좀 센세이션 했죠 그 미국에서 몇 년 전에 그런 연구를 지금까지 한 항울제 연구를 다 이제 모아 가지고 메타 연구를 해봤더니 제가 말씀드린 그 정도입니다 일반인들은 굉장히 놀랐습니다 아니 내가 병원에 가서 항울제를 먹으면네 좋아져야지 1 정도만 좋아진다는게 이게 말이 되느냐 이게 약물인데 정신과의사들이 아 역시 1 좋아지는구나 괜찮아 보는 관점이 다르죠 예 그래서
(12:41) 어쨌든 2 정도는 좋아진다 제가 아까지 말씀드리 굳이 병원을 안 오고도 그냥 친구들이나 또는 뭐 다 활동 뭐 운동을 한다든지 뭐 이런게 다 도움이 되는 된다는 이야기죠 와 가지고 상담도 하고 또 정신 치료도 받고 이러면은 뭐 훨씬 좋아지겠죠 그 약물치료에 말씀하신 심리 상담 같은 거라든가 예전에는 제가이 전기 충격 요법이라 뭐도 있었던 거 같은데 약물치료 말고 다른 방법들은 지금 현재 어떤 것들이 좀 사용되나요 말씀하신 전기 충격 요법은 과거에는 우울증에서 좀 많이 사용을 했는데 약간 이제 전기 충격이라는 뉘앙스가 별로 좋지 않했어 예 환자분들도 싫어하고 가족분들도 싫어하고 그래서 우울증인 경우는 전기 충격 육법 그렇게 많이 사용하진 않고요 이제 요사이 경 조절 술이라고 해서 tms transcal Magic 스티뮬레이션 경두개 자기자극술 자장을 형성해 가지고 그 자장을 특정 그 뇌 부위에 죄입니다 이게 tms고 해서 아마 혹시 뭐
(13:45) 병원에 가신 분들은 그런 치료를 받은 분들도 계실지 모르겠는데 그걸 해 가지고 이제 우울증을 또 좋게 하는 그런 것도 있고 또 최근에는 뭐 여러 가지 일반적인 약으로 잘 되지 않는 경우에 코에 스프레이를 뿌려서 뭐 조금 좋게 하는 되게 비싸다고 들었는데요 예 비싸기도 하고 굉장히 조심해서 사용을 해야 되는 일종에 보 케타민이란 이야기인데 어린애들 일반 어린애들 마취할 때 사용하는 그런 거거든요 해리 현상을 일으킵니다 그래서 그게 이제 에스케타민 아고 해어 사용을 하는데 꽤 비싸기도 하고 그런 경우는 이제 또 치료비도 많이 들고 여러 가지 또 부작용도 고려를 해야 돼어 어쨌든 사용을 해 볼 수가 있고요 결국 모든 방법을 사용을 해서 안되는 경우에는 그거를 써볼 수는 있죠 그러면 이제 제가 생각했을 때 우울증은 뭔가 가벼운 우울증이야기 것들이야 뭐 여러 가지 좀 좋은 활동을 한다거나 친구를 만나 가면 나아지는데 정말 어찌할 수 없는 시련들이 가끔 찾아오잖아 아까 말씀하신 그 원인들 이게 삭제가 안 되잖아요 정말 더 힘든 거는 그 원인
(14:48) 자체를 다시 바꿀 수가 없는 거니까 그 자력을 어찌해 볼 수 없는 인생의 실현은 좀 어떻게 받아들여 하는지 그리고 방금 전에 제가 말씀드린 그 내용이 교수님 입장에서 좀 공감이 가시나요 울증을 많이 다뤄보신 교수님 입장에서 예 저는 뭐 사실 우울증을 전공으로 하는 경우 경우가 아닙니다 이제 우울증 환자를 보기는 모르는데 지금 이제 김작가님 얼굴를 보니까 잘 극복하신 거 같아요 웃음도 많으시고 긍정적이신 거 같은데 우리가 이제 삶을 살아가다 보면 정말 어쩔 수없이 당하는 경우 사건 사고도 많고 뭐 굉장히 많습니다 본인이 어떻게 하기 어렵죠 어렵고 일은 생기고 굉장히 뭐 트라우마가 심하게 겪기도 하고 그래서 제 개인적인 생각은 그렇습니다 인생이라는게 희노애락을 같이 가지고 가는게 인생이지 않을까 제가 이제 60 이상 살아 본 그런 경험으로 보면 항상 좋은 일만 있을 수가 없기 때문에 그거는 인생이 아니죠 좋은 일도 있고 나쁜 일도 있고 그런데 나쁜 일이 있고 좀 불행한 일이 있을 때 이미 지나간 일이면 너무 집착을 하는 거는 좋지 않다 그러면 거기에서
(15:51) 벗어날 수가 없습니다 어떻게 보면 이미 지나간 일이니까 그걸 거울삼아 긍정적으로 잘 극복을 하시고 받아들이고 또 그거를 그게 내가 살아가는데 도움이 되게 그런 방향으로 좀 긍정적인 마인드를 가지시는게 앞으로 이제 또 다른 스트레스라는 힘든 일이 생겼을 때 극복할 수 있는 힘이 되는 거죠 회복 탄력성 이렇게 이야기를 하는데 너무 막 집착하고 이러면은 그냥 그게 용수철이 그냥 탄력성이 떨어져 가지고 흐늘흐늘 해지죠 우리가 그래 힘을 좀 키우고 그걸 개교해서 다음번에 올 수 있는 불행을 잘 극복할 수 있도록 그렇게 긍정 적인 생각을 좀 가지시면 훨씬 낫지 않을까 이런 생각이 듭니다 그리고까 우울증이나 이런 거에 도움되는 것 중에 하나가 대인 관계가 되게 중요하다 말씀을 주셔서 그러면 이제 주변에 있는 사람들 마음이 힘든 사람 위로가 필요한 사람들에게 어떤 말을 해 줘야 될까요 쓸데없는 말과 쓸데없는 행동으로 더 상처를 주는 경우도 생각보다 많거든요 말을 한마디를 딱 해 가지고 딱 고쳐지고 이런 건 사실 있을 수가 없습니다 그분한테 자꾸 뭐 어떤 이야기를 해서
(16:56) 좋게 하겠다 이런 생각은 별로 좋지 않고요 어떻게 보면 그 사람이 느낀 그 우울증이라는 고통과 고민 그걸 어떻게 상대방이 100% 이해를 하겠습니까 쉽지가 않거든요 오히려 그분이 느낀 그런 고민 고통 우울한 이유 이런 걸 좀 잘 들어 주시고 공감을 해 주시고 그런게 중요하지 정확하게 알지도 모르면 모르면서 그냥 적당한 선에서 이거이 이런게 좋아 이런게 좋아 이렇게 말씀하시는 거는 별로 도움이 안 된다 이제 듣는 사람이 입장에서도 아니 정확하게 모르면서 그냥 그죠 뭐 이렇게 이야기를 하나 뭐 이렇게 생각할 수도 있거든요 100% 옳고 그런 건 있을 수가 없 없는 거거든요 우리 환경에서 나타나는 일들이 그러니까 그거를 잘 들어 주시고 또 공감해 주시고 또 필요하면 뭐 또 용기를 북돋아주는 이야기도 해 줄 수 있지만 처음부터 그냥 너무 쉽게 그냥 아 괜찮아 좋아질 거야 뭐 이렇게 이야기를 하는 거는 별로 도움이 안 된다 이야기를 들어 주고 그 상처를 공감해 주는 것만으로도 정말 최
(17:59) 최고의 위로가 될 수 있다라는 건데 우리 중에서 마지막 질문 한 하지는 병원을 가야 되는 사람도 안 가는 이유 중에 하나가 기록이 남아서 혹시 누가 알게 되면 어떡하냐고 하는데 제가 그분한테 했던 말은 그 병원 기록은 가족도 함부로 볼 수 없어 가족 관계 증에서 그게 없 없는 이상은 본인이 외인 볼 수 없기 때문에 그냥 아프면 가서 진료 받서 아무도 모른다고 이렇게 얘기했는데 진실은 어떻게 되나요 예 의료 정보는 굉장히 개인적인 거죠 본인이 아니면 네 볼 수가 없 그렇죠 예 그리고 뭐 가족이 아도 절대 되지 않고요 일종의 정신질환에 대한 낙인 이런게 문제죠 내가 정신과에 다니면 다른 사람들은 날를 어떻게 볼까 알게 되면 어떻게 될까 예 그런 불안 있데 지금은 이제 그런 낙인이 많이 좋아져 가지고요 과거 같으면 병원에 오셔 가지고 몰래 몰래 막 남들이 알까 봐 막 숨어 있다가 복도도 켜고 뭐이 들어갔다 나오고 이러는데 지금은 뭐 다 이렇게 외래에 오셔 가지고 아주 뭐 자유롭게 그렇게 뭐 서로 이야기도 하시고 뭐 그렇기 때문에 지금은 낙인이 많이
(19:04) 좋아졌습니다 그래서 그런 것도 너무 걱정하지 마시고 언제든지 도움을 빨리 받으시면 치료가 빨리 되고 예외가 좋거든요 늦게 받을수록 우리 뇌가 어떻게 보면은 우울증이 뭐 다른 병이든간에 손상이 올 수 있는 거죠 왜냥 비정상적인 상황이 오래 지속되다 보면 그러니까는 빨리 치료를 받으시고 도움을 받으시고 본인의 어떤 앞으로의 삶을 위해서 훨씬 좋지 않을까 생각을 합니다 l [음악]
